What Living Without a Thyroid Has Taught Me About Metabolic Health

There's a small butterfly-shaped gland in your neck that most people never think about, but it's the reason I have to swallow a pill every morning for the rest of my life.
I haven't had a thyroid for 15 years. And losing it — through a complex medical journey — became the origin story for everything I now believe about metabolism, hormones, and how we fuel our bodies.

It Started When I Was 12
I was in the thick of puberty when my body started falling apart in ways that were unique from my peers. Severe migraines were pulling me out of school multiple days a week. When my family doctor finally checked my hormone levels, I got an immediate referral to a pediatric endocrinologist at Rady's Children's Hospital. The diagnosis: Graves' disease — and a particularly aggressive case of it.
Graves' is an autoimmune condition where your immune system mistakenly attacks your own thyroid, causing it to produce far too much hormone. A clinician once described it to me informally, and memorably, as my body essentially producing Speed — an accurate, yet questionable, description to give a young teen. That metaphor made sense of everything. My resting heart rate was over 100 bpm. My weight had dropped to 118 pounds at 5'7". I was caught in a relentless cycle — wired, hyperactive, then crashing into total exhaustion — day after day. Frankly, I was just mad that I had to quit playing sports.
Why Stress and Autoimmunity Are So Deeply Connected
It's worth pausing here, because I was 12. Why would a 12-year-old's immune system suddenly turn on itself?
The honest answer is that we don't fully know — autoimmunity is complex. But there is a well-documented relationship between psychological stress, major life transitions, and the triggering of autoimmune disease. Puberty is one of the most hormonally disruptive periods in human development. The body is flooded with new signals, the immune system is recalibrating, and for people with a genetic predisposition, that biological upheaval can be the match that lights the fire.
Research shows that stressful events — physical trauma, chronic emotional stress, major hormonal shifts — can dysregulate the immune system in ways that make it more likely to misfire. Stress hormones influence immune signaling, and when cortisol is chronically elevated or suddenly drops, immune resilience can break down. This is why autoimmune conditions so often emerge at recognizable stress points in life: after childbirth, after a serious illness, after prolonged burnout, or — as in my case — at the onset of puberty.
It's not a weakness. It's biology responding to pressure.
Five Years of Treatment
From age 12 to 16, managing Graves' disease was essentially a part-time job on top of school.
Monthly endocrinologist appointments. Frequent blood draws — so many that I went from having a phobia of needles to telling the phlebotomist which vein was the easiest. Daily medications to suppress my overactive thyroid (which I definitely was horrible about taking). And then, for an entire year, recurrent strep throat infections that couldn't be treated with a simple tonsillectomy, because my resting heart rate was still too high to safely go under general anesthesia. I just had to wait it out.

The most surreal chapter was radioactive iodine treatment. Basically, I took a radioactive pill aimed at destroying thyroid tissue — which sounds extreme until you've lived through endless doctor's visits for years. But the radioactivity meant I had to be isolated. I couldn't be within 10 feet of another person for a week. We rearranged my bedroom so I didn't even share a wall with my parents, to protect them from radiation exposure. At 15, I essentially lived in a bubble for a week.
After four years of this, I decided, with the support of my doctors and parents, that I wanted my thyroid out. At 16, I had a total thyroidectomy.
The Aftermath Nobody Warned Me About
I expected relief. And in some ways, I got it — the racing heart, the crashes, the chaos of uncontrolled hyperthyroidism finally stopped.
What I didn't expect was how fast my metabolism would swing in the opposite direction.
Without a thyroid, your body produces zero thyroid hormone on its own. You replace it with levothyroxine, a synthetic form of T4, calibrated to your body weight and lab values. Getting that calibration right takes time. And while my doctors were dialing in my dose, my metabolism essentially crashed.
I gained weight rapidly. This was also compounded by the fact that I was a teenager with essentially no nutrition education — and an affinity for bubble tea from the boba shop that opened directly across the street from my high school. I was not making optimal choices.
At some point, I realized my weight was creeping toward 200 pounds. And something shifted in me.
How I Got Interested in Nutrition
I didn't have a dietitian. I didn't have a nutrition program. I just started paying attention — tracking what I was eating and going to the gym (winging it with assistance from Pinterest boards).
Within a year, I lost 30 pounds. More importantly, I found something I hadn't expected: a genuine passion for understanding how the body works. My interest was imperfect and adolescent in places — restriction crept in alongside the curiosity, as it often does when young women start learning about nutrition without guidance. But underneath it was something real: a desire to understand metabolism, hormones, and why my body responded to things the way it did.
That curiosity has never left.
What Living Without a Thyroid Teaches You
When your thyroid is gone, you develop a visceral relationship with metabolic health that most people never need to think about. You learn quickly that the thyroid isn't just about energy levels. It is the master dial for your entire metabolism — regulating:
Basal metabolic rate — how many calories your body burns at rest
Heart rate and body temperature
Gut motility — how quickly food moves through your system
Cholesterol metabolism
Mood, focus, and cognition
Muscle function and recovery
When it's off — even slightly — everything feels off. Sleep suffers. Weight shifts. Brain fog moves in. You feel either wired or flat, and sometimes both within the same day.
Living with that reality for 15 years has made me deeply attuned to the signals most people are taught to dismiss or push through.
What Actually Supports Thyroid Health Nutritionally
This is where I want to be honest with you — because the thyroid nutrition space is full of noise. Elimination diets, fear-mongering about goitrogens, miracle supplements. Most of it isn't supported by evidence, and some of it actively gets in the way.
Here's what the research actually points to.
The nutrients that matter most:
Selenium is essential for converting T4 into the active T3 your cells actually use, and for protecting the thyroid from oxidative damage — get it from Brazil nuts (just 1–2 daily is enough), sardines, tuna, and eggs.
Iron is required for the enzyme that produces thyroid hormone in the first place, making deficiency a surprisingly common culprit behind persistent fatigue — prioritize red meat, lentils, spinach, and tofu, ideally paired with vitamin C to improve absorption.
Vitamin D supports the immune regulation that underlies autoimmune thyroid disease — fatty fish, egg yolks, and fortified foods help, but if you're deficient (worth testing), a supplement is usually necessary to actually move the needle.

The dietary pattern that shows up consistently in the evidence:
Observational research points toward a Mediterranean-style diet as broadly supportive of thyroid health — rich in selenium, polyphenols, omega-3 fatty acids, fruits, vegetables, and legumes, and associated with reduced oxidative stress markers.
Western-style diets high in red and processed meat, by contrast, have been linked to increased thyroid autoimmunity. It's not magic; it's consistent delivery of the antioxidants, healthy fats, and micronutrients the thyroid pathway depends on.
What you probably don't need to worry about:
Gluten-free and dairy-free diets don't provide meaningful thyroid benefit unless you have a confirmed sensitivity or celiac disease — in which case, removing gluten may genuinely improve levothyroxine absorption. For everyone else, it's an unnecessary restriction.
As for cruciferous vegetables and soy — eat them. Like most things in nutrition, dose matters. The compounds in these foods that can theoretically interfere with thyroid function are subject to dose-dependent responses, and actual harm is associated with very large, sustained intake — not a normal serving of broccoli or edamame. Moderation, as always, is the governing principle.
The Philosophy I've Landed On
Everything I've experienced has shaped the way I think about supporting metabolic health — and the way I want to help other people approach it.
Restriction isn't the answer. It was never the answer. My body has been through enough — a hyperactive immune system, years of medication, radioactive isolation, surgery, a crashing metabolism — and what it has consistently responded to is nourishment, not deprivation.
The approach I believe in is nutrition by addition: asking what your body needs more of, rather than starting with what to take away. More protein to support thyroid hormone conversion and muscle metabolism. More of the micronutrients the thyroid pathway actually depends on. More consistency. More trust in your body's signals.
Hormones and metabolism are not your enemies. They're simply information. And the more fluently you can read them, the better you can support the body that's carrying you through this life.
